M.A.R.C.H/Tactical Combat Casualty Care Training (8HRS)

Description 

Demonstrate comprehension of M.A.R.C.H. principles (Massive hemorrhage, Airway, Respirations, Circulation, Head injury/Hypothermia) and competency at life-saving skills.  This course is taught with the latest standards of care recommendations by the Joint Trauma System (JTS), American College of Surgeons, and the Committee of Tactical Combat Casualty Care (CoTCCC).

Note:

This is mostly an indoor class (in the classroom) with a lot of presentations and hands-on exercises. Weather permitting, the instructor may take the students outside for drills and exercises.  There will be NO firearms/ NO live fire involved in the course at all.

Learning Objectives:

  • Recognize massive (life threatening) hemorrhage.

  • Demonstrate competency using commercial and improvised tourniquets

  • Recognize common causes of airway compromise.

  • Demonstrate how to position someone in the Recovery Position.

  • Recognize signs of difficulty breathing.

  • Basic MARCH

  • Demonstrate the physical examination to identify penetrating chest trauma.

  • Demonstrate the placement of an occlusive dressing for penetrating chest trauma.

  • Differentiate non-life-threatening hemorrhage from massive bleeding.

  • Demonstrate the proper placement of a trauma dressing.

  • Demonstrate wound packing techniques when use of a TQ is not feasible.

  • Describe the adverse effects of hypothermia on trauma patients.

  • Demonstrate the proper utilization of an emergency casualty blanket to prevent hypothermia.

Equipment Needed

  • Task Trainers and medical supplies for training will be provided

  • Current Medical Items Commonly Worn (if you don’t have your own, it will be provided)

  • Tactical Gear Commonly Worn

  • Medical SOPs if available

  • Knee pads (optional, but recommended)

  • Water for hydration

  • Notepad

IINSTRUCTORS BIO:

-David Smith-
David retired after spending 27 years as U.S. Air Force Independent
Duty Medical Technician (IDMT) and a national flight paramedic
(CCEMT-P). He has played a key role in medical planning, direction,
treatment, and support during his 12 deployments to places like
Afghanistan, Iraq, Africa, Haiti, Ecuador, and the Philippines. David
spent 5 years leading and working in a clinical trauma training program for doctors and medics assigned to the Air Force Special Operations Command at both John Hopkins Burn Center, Emergency Department and the University of Maryland’s Shock Trauma Center in Baltimore, MD. He is a trained EMS Educator and has been a consultant, for multiple companies for the past 14 years. His last position he was the Director of Operations/Training for MOTC which overseas more than 400 federal agents that have collateral duties as Tactical Medics (NREMT). He is
instructor-certified in Tactical Combat Casualty Care (TCCC), Pre-
Hospital Trauma Life Support (PHTLS), Advanced Cardiac Life Support (ACLS), Basic Life Support (BLS), Pediatric Advanced Life Support (PALS), and Pediatric Education for Prehospital Professionals (PEPP). He also has specialized training in Mobile Force Protection and Force Protection Tactical Off-Road Course and Anti-Terrorism

Lead instructor David Lang started his journey in the 82nd Airborne as a medic deployed to Operation Enduring Freedom, Afghanistan. Furthering his pre-hospital knowledge by assessing for Task Force 160th and attending the Special Operations Combat Medic Course. Later diving deeper into Flight Medicine and Critical care he obtained his Flight Paramedic Certification and Critical Care endorsements. He later went on to Instruct at the Flight Paramedic School house and Expeditionary Medical Course at the Army’s Fort Sam Houston (home of Army Medicine). Then moved on to become an Operations Chief for the Walter Reed Simulation and Training Dept, specializing in high-risk low acuity training for the DOD’s medical school. Currently holding a position at the State Departments Operational Medicine Directorate.
